Successful Treatment of
High Perianal Fistulae by Simple Coring (15 Years Experience)

Surgical treatment of high
perianal fistulae represents a challenge to the surgeon . Lay open can not be
used at all, coring with mucosal flap failed and recurrence is the rule, seton
suture takes very long time . For me I used since 15 years ago what we can call
it " simple coring " in which I do not close the internal opening of the fistula
by mucosal flap, no recurrence of any case occurred either low or high fistulae
after this very long time of follow up .
I think in my simple method
the presence of the internal opening unclosed acts as a drainage for the pus
which is formed in the internal half of the tract and causes recurrence if the
opening is closed as pus here has to go through the tract to the external
opening which causes recurrence.
In my method 2 openings are left for drainage of the tract in both directions so
no recurrence.
30 cases were studied: 16
were low fistulae, 14 cases were high fistulae, 7 Of them were primary and other
7 were recurrent .
