(S) Plasty Instead of (Z) Plasty as a New Surgical Treatment of Post Burn Contracture

 (z) Plasty is being used now in treatment of post burn contracture, this method has many draw backs as there is no actual increase in skin surface area and the increase is due to undermining of the skin. This leads to make multiple incisions, also strong traction may lead to necrosis of the apices of skin triangles.

For these reasons I constructed new technique which I call it (S) plasty. In fact the incision is done as so (). It is made perpendicular to the line of contracture. By this incision 2 adjacent rectangles are produced, both are separated and put facing to each others, then sutured to each other and adjacent skin. The final picture will be so (H). by this new technique, there is actual increase in skin surface  area in the direction of contracture as we transfer the redundant skin on each side of the contracture to the line of contracture. So only one incision is done and no fear of necrosis of any piece of skin.

The study was done on 12 patients, 10 females and 2 males, 9 cases were contracture opposite elbow joint, 3 cases opposite ankle joint, all cases healed within normal time without complications or recurrence.
